We want to be able to share candid images of the reality of being an adult and living with an IDD, especially as we build a community through 22plus.

By signing this form, I grant 22plus permission to use photographs and/or video recordings of me (or my loved one/dependent) taken at 22plus events for promotional and educational purposes. I understand that these images may be used in:

Marketing and promotional materials
I acknowledge that participation is voluntary and that I will not receive compensation for the use of these images. If at any time I wish to revoke this permission, I may contact 22plus in writing, and reasonable efforts will be made to discontinue future use.
